As a Life Care Planner you are faced with a challenge when working with an individual who has a severe brain injury; the associated cognitive, physical, behavioral and psychological issues will create lifelong problems. You may be working to develop a Life Care Plan or assist the legal defense team to secure an adequate settlement that addresses the person’s true needs, and provides the appropriate care and resources they need to carry them through the process of aging with a disability. In all instances, you will be looking for brain injury rehabilitation resources and specialized care providers who have the skills, competence and experience to provide an appropriate level of care and program of services for your client.
The Neurologic Rehabilitation Institute at Brookhaven Hospital will partner with you to create and implement cost-effective and outcome-oriented programs for your client with a brain injury and severe neurobehavioral, cognitive and physical deficits. The Neurological Rehabilitation Institute programs begin with hospital-based care and then extend into the community. Our programs offer a range of services, from Intensive Neurobehavioral Rehabilitation to Complex Care for slow-to-recover individuals. We also offer community-based rehabilitation in our Transitional Living Center, or through Lifespan, our Supported Living Program. In each of our programs, provide quality care through a team of rehabilitation professionals and a specially trained nursing staff. Over the years the NRI program has earned a reputation of providing quality outcome-based brain injury rehabilitation to individuals with severe problems that have created difficulty in traditional rehab programs, at home, with family caregivers or in extended care placements. Often these cases have been referred to us in crisis and some individuals have traveled to us from foreign countries as well as throughout the U.S. The Neurologic Rehabilitation Institute offers a unique range of services for your most difficult-to-place clients.
