Refer A Patient

If you'd like to refer a patient to the Neurologic Rehabilitation Institute at Brookhaven Hospital, please call (888) 298-HOPE [4673] and ask to speak to a TBI service coordinator. We may ask you to provide faxed medical records to (918) 438-8016. Your referral will be thoroughly evaluated and will receive a follow-up response from a brain injury case manager within 24 hours of your contact.
